  • Specific Example of Failure: The most recent complaint (Case #00517955, CRN 43250314998, logged 15/07/2025) is already marked as "Investigation Complete" just two days after the automated acknowledgment was received (Log #590).
  • Identified Pattern of Systemic Failure:
    1. No Victim Engagement: Cases are being marked "Complete" without any substantive contact, follow-up, or notification of outcome provided to me, the victim.
    2. No Real-World Resolution: The "Complete" status does not reflect any actual resolution of the harassment, threats, or abuse. The harm continues unabated.
    3. Dereliction of Duty: This pattern demonstrates a systemic failure by TVP to investigate crime, protect a vulnerable victim, and uphold the Victims' Code of Practice. It is evidence of institutional neglect on a massive scale.
• My Perception of Purpose & Impact:
  • This is the definitive proof of the "procedural suppression" and "institutional gaslighting" I have been alleging.
  • It proves that my complaints are not being investigated; they are simply being processed and closed administratively to create a false impression of efficiency.
  • This "Catalogue of Failure" is the primary exhibit for my complaints to the IOPC, the PCC, my MP, and my High Court actions. It proves, with their own data, that the system is broken.
