I submit this affidavit in good faith, under full liability and penalty of perjury, as a sworn record of bereavement-related injury and its systemic aggravation by the institutions named herein.
I, :Waseem: Malik, am the biological son and next of kin to the late Mrs. Munwar Sultana Malik, who passed away on 10 January 2025 at Wexham Park Hospital, Slough. I was the sole organiser, financial contributor, and legal point of contact for her funeral, burial, and estate affairs.
We shared a deep familial bond, characterised by mutual reliance, cultural preservation, and emotional intimacy. Her passing marked the collapse of my immediate support structure, and her absence continues to affect my physical, psychological, and spiritual wellbeing.
In the absence of institutional support or community protection, I bore the emotional trauma and procedural consequences of her death in isolation. This relationship was not merely familial — it was foundational to my identity, resilience, and survival.
Following the death of my mother on 10 January 2025, I entered a state of acute psychological trauma. Rather than being afforded a period of peace to mourn, I was immediately subjected to:
